DINO JIU JITSU PARTICIPATION WAIVER & RELEASE OF LIABILITY

This form must be completed and signed before participating in any activity at Dino Jiu Jitsu. By signing this agreement, you are waiving certain legal rights. If the participant is under 18, a parent or legal guardian must sign.

1. Assumption of Risk

I understand that Brazilian Jiu-Jitsu and related activities involve physical exertion, close contact, and inherent risks. These risks include but are not limited to: joint injuries, sprains, fractures, choking hazards, concussions, skin infections, equipment failure, and injuries caused by other participants or instructors. I knowingly and voluntarily accept all risks, whether foreseeable or not, including serious injury or death, for myself or my minor child.

2. Health Acknowledgment

I affirm that I (or my child) am physically fit to engage in strenuous physical activity. I agree to disclose any relevant medical conditions and acknowledge that it is my responsibility to monitor our health and consult a physician prior to participation. I will not attend class while contagious, sick, or recovering from illness without medical clearance.

3. Release of Liability

In consideration of being allowed to participate, I, for myself, my heirs, personal representatives, and assigns, release and discharge Dino BJJ LLC, its owners, staff, instructors, volunteers, and agents (the “Released Parties”) from any and all liability, claims, demands, or causes of action arising from any injury, loss, or damage to person or property, including death, arising from participation or presence on the premises—whether caused by negligence, acts of others, or otherwise.

4. Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties against any claims, demands, damages, costs, or legal fees incurred due to participation by me or my child. This includes claims brought by other parties related to my (or my child’s) conduct.

5. Infectious Disease Waiver

I acknowledge the contagious nature of COVID-19, staph, and other transmissible conditions. I voluntarily assume the risk of exposure and agree that Dino BJJ LLC is not liable for any resulting illness or medical consequences.

6. Photography & Media Release

I grant Dino BJJ LLC the right to use photos, videos, or audio recordings of me or my child taken during training for any lawful promotional purpose. I waive the right to inspect or approve the final product or receive compensation.

7. Rules & Conduct

I agree to follow all gym rules, instructions, and safety protocols. Dino Jiu Jitsu reserves the right to revoke access at any time for misconduct or failure to comply with policies, without refund.

8. Jurisdiction & Severability

This agreement is governed by the laws of the State of Texas. Any disputes will be resolved exclusively in a Texas court of law. If any portion of this agreement is deemed unenforceable, the remainder remains in full effect.

Acknowledgment:
I understand the legal impact of signing this document. I certify that I am signing this voluntarily and with full understanding that I am waiving my right to bring legal action against the Released Parties.

DINO JIU JITSU MEMBERSHIP AGREEMENT

PAYMENT TERMS  

• No sign-up fees. 
• Your membership starts on the day you sign this form and will be prorated for the remainder of the month.  
• Your monthly billing date will be on the 1st of each month. Your dues will be automatically charged to your card or checking account on file on the 1st of each month.  
• You may put your membership on hold as needed or as personal circumstances require you to do so. Life Happens, we get it.  
• Cancel any time. Just tell us why and how we can improve. You may be required to buy Marie a dozen donuts.   

EQUIPMENT POLICY  

• No member or visitor is required to purchase any equipment from Dino Jiu Jitsu.  
• All members are required to wear clean, sanitized, and proper attire for the perspective training class.  
• Members with their own Gi are encouraged to purchase a Dino Jiu Jitsu patch, but not required. You would be a lot cooler if you did.  
• Members / Visitors without a Gi can borrow a loaner on a class by class basis for up to one week subject to availability.   
• Members / Visitors will not be allowed to borrow or loan rash guards from Dino Jiu Jitsu. Feel free to ask a fellow student though.  
• Mouth guards are a really good idea. Dentist work aint cheap.  
• Forget your belt…..May the universe have mercy on your soul. 

PAYMENT AUTHORIZATION 

I authorize Dino Jiu Jitsu to charge my credit card for monthly dues and any past due amounts on my account. This authorization is as valid as a physical signature.
